Born: 7th October 1892, Westminster, Middlesex.

Baptised: 18th December 1892, at St. John the Baptist Church, Great Marlborough Street, Westminster. Parents: Charlotte & Samuel Collins, a Fireman with the London County Council, family home – the Fire Station, Great Marlborough Street, Westminster.

Died: 5th June 1916; age 24; Killed by mine explosion – Orkney’s.

Occupation: a Butcher.

Enlistment Location: Portsmouth, Hampshire. Signed up for 12 years on the 7th October 1910. Height 5ft 5 1/8ins, fresh complexion, dark brown eyes and dark brown hair.
